At 7:19 on January 8th, Zhang De (a pseudonym), 25 years old, received a phone call from the Licheng Defence Office in Jinhu County, Jiangsu Province, and asked him to "register". At this time, he discovered that the circle of friends was an overwhelming "expired vaccine" message.
Just one day ago, his son, who had just completed 100 days, was taken to the Jincheng County Licheng Defense Institute to receive polio vaccine (hereinafter referred to as "polio vaccine"). “At the time, the doctor gave the child a few drops and made another shot.†He recalled, “The whole process is very fast. We are taking care of undressing the children and not paying attention to the date of production of the vaccine.â€
After hanging up the phone, Zhang De looked through the record and found that the son's oral polio vaccine was the expired vaccine with the batch number "201612158". "Now the whole family is very anxious, and I don't know where to take the children to check it." Zhang De said.
In the past few days, hundreds of parents like Zhang De in Jinhu County have been in panic.
On the afternoon of January 10, the relevant person in charge of the Propaganda Department of Jinhu County Committee reported that 145 infants and young children had been diagnosed with expired polio vaccine. According to the report, the relevant staff of the Lihu Health Center of Jinhu County and the Jinhu County CDC of the vaccine management department have “management confusion, job dereliction of duty, and supervision failureâ€. At present, the leadership team of the Jinhu County CDC and related departments have been completely dismissed. "The new leadership team is in place and will meet tonight to discuss the next step."
Expired vaccine that is "missed"
Later, Zhang De learned that on January 7, a parent working in the hospital found that the vaccine expired when he took the child with a polio vaccine. The batch of polio vaccines is 201612158, and the manufacturer is Beijing Beishengyan Biological Products Co., Ltd., valid until December 11, 2018.
According to reports, the batch of polio vaccine is a bivalent live attenuated vaccine (OPV), commonly known as "small sugar pill", which belongs to the state-provided type of vaccine, which is distributed and recycled by the CDC. At present, parents have formed an “expired vaccine victim groupâ€, and nearly 1,000 parents have joined.
After the incident, the Jinhu County Party Committee and County Government launched a major incident emergency response mechanism, convened an emergency meeting overnight, set up an incident handling headquarters, and was investigating the incident. The County Commission for Discipline Inspection was also involved in the investigation. At the same time, the county has organized a network inspection of all medical units in the county to completely eliminate expired vaccines, expired drugs and expired consumables.
According to a local parent, Ms. Cai, on the afternoon of January 7, the deputy magistrate of Jinhu County organized the head of the county health care committee, the disease control center, and the health center to meet with the parents, and invited the Jiangsu Provincial Center for Disease Control and Prevention. Nanjing Children's Hospital experts answered questions on the spot. Experts told them, "Most of the response caused by the vaccine is within 14 days. To be proper, it is recommended to extend the observation period to 40 days and strengthen follow-up."
Experts said at the Q&A site that the hospital should not have mixed batches of vaccines for a long time. "This is easy to cause confusion. You should check the activation time and mixed time of each batch as soon as possible."
On January 10, the relevant person in charge of the Propaganda Department of Jinhu County Committee reported that the expired polio vaccine was distributed on May 9, 2017 by the Huaian City CDC to the Jinhu County CDC, Jinhu. The county CDC delivered the cold chain to the Licheng Health Center on May 18 of that year.
The report also stated that on December 27, 2018, the Jinhu County CDC notified the inoculation units to send the expired vaccine to the county CDC and concentrate it on the municipal CDC for unified destruction. However, the Licheng Health Center did not submit an “expired vaccineâ€, and the county CDC did not continue to report.
Vaccine use
China Youth Daily·China Youth Online reporter found that after the incident was exposed, many parents in Jinhu County went to check whether their children had expired vaccine. Some parents found that the use of expired vaccines not only appeared on the polio vaccine with the batch number "201612158".
"My child's 4 needles expired, 3 needles can not find the record information." 32-year-old Luo Mei (a pseudonym) has been insomnia for two days since she discovered that her daughter had a problem with the vaccine.
The reporter found that Luo Mei’s daughter was inoculated with two batches of “100130320-2†on April 6 and May 6, 2015. The batch number is valid until March 29, 2015; 2015 The leprosy vaccine with the batch number "201308175-1" was inoculated on July 27, 2009, and was valid until February 17, 2015. On September 14, 2015, the JE (attenuated) with the batch number "201304047-2" was inoculated. The vaccine is valid until October 11, 2014.
"The short period has expired for 8 days, and the long period has expired for nearly a year. Now I am afraid of thinking about it," Rome said.
This is not a case. The 29-year-old Ms. Tang checked her son’s vaccinated batch number online and found that many of the vaccines injected in 2011 did not have information.
Ms. Tang found that one of the “Brain A+CI†vaccines with the batch number “20120101-1†was expired. The online inquiry expiration date was January 1, 2014. “My son’s vaccination time is January 20 of the current year. ".
It is worth noting that many parents said that the registration of the child's vaccine record is blank, which prevents parents from checking whether the vaccine has expired.
Ms. Cai, 32 years old, said that on December 12, 2018, her 4 year old child was vaccinated with polio, and the doctor did not record the vaccine lot. Although the child did not have adverse symptoms, "but the family is still worried, I plan to take the children to the big hospital to check these days."
In addition, the phenomenon that the vaccine records this information registration error also exists. A parent found out that BCG, which was vaccinated at the hospital after the birth of the child, also expired. Later, she found the hospital to verify that the anti-insurance office copied the “2017†into “2016†when it transferred the information of the hospital BCG, which caused misunderstanding.
"This can only indicate that vaccine managers and vaccinators are derelict." According to a disease control expert, vaccine managers must undergo professional vaccination training in order to be certified. According to the regulations, vaccine administrators need to check the expired vaccine regularly, and also need to carefully check the vaccine batch number and expiration date when the vaccine is out of the warehouse. In addition, when vaccinators use the vaccine, they also need to “three check seven pairs†according to the regulations.
The problem of vaccine management confusion in hospitals has been discovered before, but has not been rectified
Wang Yuedan, deputy director of the Department of Immunology at Peking University School of Medicine, said that in theory, there is no expired vaccine in the circulation of vaccines, and the law does not allow expired vaccines to enter the inoculation process. The recovery of a type of vaccine and a second type of vaccine is different. One type of vaccine is handled by the CDC, and the second type of vaccine is recycled by the company.
Wang Yuedan also said that there should be a regulatory mechanism for vaccination sites. One type of vaccine is under the responsibility of the CDC management system. The CDC has a ledger. The number of vaccines issued and vaccinated at each inoculation point is electronically recorded, and the number of vaccinations corresponds to the number of vaccinations. In the process, it is impossible to rule out the loss during the inoculation process, the vaccine is broken or other problems, and some vaccines have not been reported in time.
Wang Yuedan said that the harm of expired vaccine lies in two aspects. First, the vaccination titer cannot be guaranteed, and the vaccine has an expiration date. After the expiration date, the immune protection of the human body may not meet the requirements. Second, because the vaccine itself is also a protein, it may be degraded if it expires, such as preservative failure and bacterial contamination.
According to the relevant person in charge of the Propaganda Department of Jinhu County Committee, in 2018, the Jinhu County CDC conducted two inspections on the Licheng Health Center and found a series of problems. The inspector found that the Licheng Health Center had a large amount of inoculation on a single day, the vaccine management was confusing, the actual use of the batch number and the batch number of the inbound and outbound books were inconsistent, and the rectification notice was issued. Until December 15, 2018, the Jinhu County CDC re-inspected the above problems, but the above problems have not been rectified, but the county CDC did not promote the rectification or report.
According to the report, on December 28, 2018, when Huai'an organized a vaccine-related work inspector, it also clearly pointed out that the vaccine, the outpatient log and the vaccine system of the Licheng Health Center were in terms of the number of vaccines, manufacturers, batch numbers, etc. There are inconsistencies and other issues, and require rectification within a week. However, the health center has not been rectified, and the Jinhu County CDC has not supervised the rectification and implementation, so that the expired vaccine has not been reported, turned in, and continues to be used.
